These forms must be filled out and collected by the Team Manager and confirmation of signed Fair Play Codes needs to be given to the Division Coordinator. All players must fill out a Fair Play Code for Players and it is recommended to fill out a medical information sheet. Coaches and parents must fill out the appropriate Fair Play Code.

This is a form for you to fill out with the contact information from each player including jersey numbers as well as coaches, manager and phone parent info.

This is a basic form for keeping track of your team finances. Give this to your Treasurer for balancing your team account. You will need to review this with the Treasurer and send in a copy to your Division Coordinator after the Seeding Round and at the end of the season.
