Forms and Policies

The following forms are used by the McKnight Evaluation Committee to score your child during season evaluations.  Please review them with your child in order to help them understand what the committee is looking for and what they will be scored on.

In the event that you would like to appeal your player's team placement, please completely fill out the form below and submit it to Sam Livingston - secretary@mcknighthockey.com by (TBA).  Absolutely no appeals will be accepted after this day.
